COMMUNITY HEALTH (CONTINUED)

Considering that the family is the foundational unit of society, CHFESDI provides individual and group parenting education programs to promote the healthy development of children. In terms of social cohesion and tolerance, initiatives aimed at encouraging openness and acceptance of others—specifically addressing issues related to body image, interpersonal relationships, and healthy sexuality—are among the subjects that the Community Health Centers (CHC) promoted by CHFESDI will tackle.

Providing coordinated and integrated care within the community, closer to home. The CHC system is a proven model of primary healthcare that has been successful in other countries, such as Canada. It brings together primary healthcare services and a wide range of health promotion and community development services under one roof.

Contributing to disease prevention by addressing the social determinants of health. There will be interdisciplinary teams that will not only address individuals’ health issues but also create programs and services aimed at mitigating the social and environmental problems that affect the health of the communities served. All services will be carefully tailored to meet the diverse needs of these communities.

Contributing to disease prevention. The community health centers will help transform the current healthcare system into a patient-centered system focused on preventing avoidable diseases and ensuring the sustainability of the system. They will play a key role in bringing about real changes.

Promoting health and community engagement.

As the name suggests, a community health center is a primary care structure that reaches where no other partner can. Thanks to its proximity to the community, the community health centers will contribute to helping people stay healthy. The CHCs address health issues using a holistic approach, recognizing that health is not just a state of physical, psychological, or mental discomfort, but also encompasses socio-economic, cultural, financial, and environmental aspects, along with housing and employment. Through the CHCs, CHFESDI aims to address social issues such as: unemployment and underemployment, education, cultural promotion and integration, housing and the environment, early childhood development, and family well-being, all of which contribute to the good health of individuals.

Finally, the CHCs will position themselves as primary partners within their communities, helping each person to live in a healthy community. They will also assist urban and rural communities in taking control of their own health through the adoption of disease prevention methods, well-being, and social cohesion.
